What the inspector wrote

On 4/08/2025 Licensing Program Analysts (LPAs) Isabel and Hanna conducted an unannounced subsequent complaint inspection to deliver the finding for the above allegation. LPA met with Facility liaison and completed a walk through the facility. Upon arrival LPA observed 11 children and staff providing care and supervision. During the course of the investigation, LPAs gathered documents relevant to the complaint allegation. Based on the evidence, record review, although the plan was to utilize the school age van for transportation the van does not have a the required approval. Therefore staff with a valid Driver's license and auto insurance transport the school age children to and from school with parent consent(transportation consent form). This allegation is deemed Unsubstantiated, a finding that is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged allegations valid. No deficiency was cited for this investigation. An exit interview was conducted, a copy of this report, appeal rights and a notice of site visit were provided to facility.

File review, documentation, and interview. It was disclosed six out of eight staff do not have the complete required immunization on file with full name and date of birth. According to staff medical records with immunization are being requested from Doctor's office. This poses a health and safety risk to children in care
