1596.7995.(a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.This requirement was not met by evidence by... File review, documentation, and interview. It was disclosed six out of eight staff do not have the complete required immunization on file with full name and date of birth. According to staff medical records with immunization are being requested from Doctor's office. This poses a health and safety risk to children in care
