What the inspector wrote

On Wednesday, January 21, 2026 Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up in regards a self-reporting incident that occurred on October 31, 2025. LPA met with site supervisor Candice Correa. On November 5, 2025, Palmdale Regional Office received an unusual incident report stating child #1 personal rights might have been violated. During this inspection, LPA Rivera observed present staff fingerprinted cleared and facility to be within ratio. LPA observed a total of 16 preschool children and staff #1, staff #2, staff #3 and staff #4 present providing care and supervision. LPA interviewed two children and teacher. Based on information provided and interviews conducted the incident does not appear to have been the result of any violation of Title 22 regulations, therefore, no deficiencies were issued. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with site supervisor Candice Correa. A copy of the report was also provided.
