On Thursday, December 18, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Director Hermineh Azizian. The purpose of the inspection was to conduct a follow-up unannounced case management for an Unusual Incident Report (UIR) received at Palmdale Regional office on 10/20/2025 regarding parent of Child #1 (C1) stating that C1 received a bump on the head at the facility. LPA Cha disclosed the purpose of the inspection to the Director. Upon arrival, there were at total of 21 children and six teachers providing care and supervision. The current days and hours of operation are Mondays through Fridays; 7:30am-4:30pm for toddler program and 7:30am-5:30pm for preschool program. During the visit, LPA Cha interviewed three staff. LPA Cha completed a safety inspection of the facility. No safety concerns were observed. Based on LPA observations, no further follow-up needed. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with Director Hermineh Azizian.
