What the inspector wrote
On November 10, 2025, Licensing Program Analyst (LPA) Calloway made an unannounced inspection to the above facility for the purpose of conducting an Annual Random inspection. LPA met with facility representative who granted access. LPA and representative toured the facility inside and outside. LPA observed seventeen (17) day care children in active play and six staff. The facility is open from 6:30 to 11:00 AM. Monday-Friday. All employees 18 years or older that are working or volunteering have a Criminal Record Clearance (DOJ/FBI) and Child Abuse Central Index Clearance and are associated with the facility.
Physical Plant:
There are five (5) clean and safe classrooms for infants and preschool, the telephone service, heating, ventilation, and lighting are adequate. The two infant classrooms had three cribs, three changing tables, and diapers and wipes. The daily schedule and activities were posted. The preschool classroom there are cubbies for children's belongings that are labeled with their names and photos in each classroom. The furniture, books, equipment, toys, and materials were appropriately aged and in good condition. There were activity rugs inside the room for play. Representative stated the rugs are cleaned weekly. The drinking water is available inside the classrooms in the form of water dispensers and labeled or disposable cups. Representative stated the cups are cleaned daily. The napping equipment are cots and there was enough for all children. Representative stated the bedding is stored separately and washed weekly. First Aid Kits were in each classroom in a red backpack. There was no Emergency Survival Kit. Trash cans have secure lids. The temperature inside the room
was comfortable. Representative stated, the fire extinguishers, smoke detectors and carbon monoxide detectors throughout the day care center are all operable and there has not been a fire inspection for this year.
Bathrooms:
There are small toilets and handwashing sinks that are functioning properly and at the appropriate height. LPA did not observe any accessible hazards.
There was soap, toilet paper, and paper towels available. The staff restrooms are in the infant room in the corner labeled.
Kitchen:
There is a fully equipped kitchen with refrigerator, freezer, stove, oven, prep counter, and sink. The kitchen was clean and there were no accessible hazards observed. The facility provides breakfast, lunch, and afternoon snacks. The menus are posted on the refrigerator in the kitchen. There is a confidential allergy list posted on the wall in the kitchen. LPA observed an appropriate amount of food and snacks. The chemicals and cleaning supplies are kept separate from the food and are locked under the kitchen sink with safety latches. LPA observed 1% milk and 2% milk. Representative stated the 2% milk was purchased incorrectly.
Outside:
Outdoor play equipment was inspected for safety, cushioning material, good repair and is age appropriate. LPA observed the infant yard with plastic play structures with slides, bike cars, and grass for cushioned surface. The fencing was vinyl and secure around the whole playground. The preschool yard had a large play structure and a climbing apparatus securely anchored in the ground with trees for shade and artificial turf for cushioned surface. Drinking water is available in the form of a portable water jug and disposable and labeled cups. Representative stated there are no bodies of water on the premises. LPA did not observe any large bodies of water. There was a large water table that was empty. There are two storage sheds that were locked. The fencing is wrought iron and chain linked with privacy screening, observed locked and is secure around the entire playground. Both of the playgrounds were observed to be free of debris.
Other Review:
Children and staff files were reviewed. Current Pediatric CPR/First Aid (expires: 7/2027). Representative stated transportation is not provided. Disaster Drills: Fire Drill was conducted: 7/21/2025 Earthquake drill was not conducted. Representative stated the facility fire inspection has not
been conducted and should be by the end of the year. The child/parent roster was incomplete with all required information. Six children were missing physician names and phone numbers. The staff/ child ratios were observed to be compliant; the sign in/out sheets were paper and legible. Representative stated the children are inspected for illnesses as they arrive.
There is a separate isolation area for ill children in the site supervisor's office until the parent can pick up the child. Representative stated, the medications are stored in a small wall cabinet locked. A review of medication policy states that prescription medication is administered only with parent's written permission. The management staff administers medication and documents the dosage, date, and time on a log. Medication is stored in the locked cabinet. All medication is properly labeled and stored in its original container. Staff interview was conducted with the Representative and LIC 125 form was discussed.

All representatives are responsible for knowing the regulations when providing care. All employees should know their role and responsibilities of being a Mandated Reporter. Training is renewed every two years at: mandatedreporterca.com. All adults 18 years and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a Criminal Record Clearance or Exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-
CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers that were constructed before January 1, 2010, to test their water (used for
drinking and food for lead contamination before January 1, 2023, and then every 5 years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at:

sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. State law prohibits baby walkers, bouncy seats, exersaucers, bassinettes or any other items that fall into that category. All infants are sleeping on their backs unless specified with LIC 9227 form and no blankets, toys, or pacifiers attached. Conduct 15-minute nap monitoring and a napping log is kept daily for each infant.
Facility representative was informed of the MyChildCarePlan.org website, a
consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Exit interview was conducted and a copy of this report was read, a Notice of Site Visit (LIC 9213), this report, and Appeal Rights were provided to Marianne Haddad, Facility Representative. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
