What the inspector wrote

On June 2, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on May 27, 2026. LPA met with Arielle Guillen, the facility director as the representative, toured the facility inside and outside with the representative, and observed sixty-three (63)-day care children and eight staff in active care. All adults 18 years and over have Criminal Record Clearance and are associated with the facility. On May 20, 2026, the reported information involved a child (C1) that was running from the tree area towards the play structure during outdoor play when they lost their balance and fell to the ground. C1 sustained an injury to their left knee initially then when they got home, they complained to their parents of left shoulder pain, and the parent took C1 to the urgent care where it was discovered C1 sustained a fractured left clavicle. During this inspection, LPA conducted interviews with all relevant parties involved. At 3:30 PM. The children went outside to play. At 3:43 PM., LPA completed a safety inspection of the playground and observed it to be locked and secure and there was no debris on the ground from the tree area to the play structure. LPA observed five staff and fifty-eight (58) children outside in active play on the playground. LPA measured the distance with a roller stick and it measured forty-three (43) feet from the tree to the wood chips entering into the play structure. There was a tether ball area with two poles to the left an additional two feet distance from the wood chips area. Based on interviews, on May 20, 2026, the staff in the area that was assigned acknowledged they did not witness C1 fall. Staff stated they were assigned to the mega area on the day which is required to oversee the entire playground and all of free play and not one specific section therefore limiting their supervision capabilities. Although the staff took appropriate action accessing the child, notifying the parents, and Community Care Licensing (CCL), the care and supervision violation did occur. There is one Type B deficiency cited during this inspection for 101229(a)(1) CCR, See the LIC 809D page attached to this report. Exit interview was conducted, a copy of this report was read, a Notice of Site Visit, and Appeal Rights were given to Arielle Guillen, director, at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting may result in a $100 civil penalty.
