What the inspector wrote

On January 23, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on January 21, 2026. LPA toured the facility with the representative and observed no school aged children in care during the inspection. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. On January 21, 2026, the reported information involved Child (C1) sharing chocolate peanut candy with Child (C2) and C2 stated their tongue felt funny and numb. Staff notified the parent immediately who arrived and administered allergy medication to their child (C2). C2 became ill and started to vomit, then the parent took C2 to the emergency room for further review and medical treatment. During the inspection, LPA conducted interview with relevant parties involved, reviewed files, and toured the classroom and kitchen. Based on interviews and documentation this reported usual incident needs further investigation. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, were given to Marianne Haddid, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
