What the inspector wrote

On 06/25/2025, Licensing Program Analysts (LPAs) Jonnisha Culbert and Javier Duran conducted a required-3 Year annual inspection at the facility noted above. LPAs met with licensee, Lourdez Muniz provided them with an Entrance Checklist for Child Care Centers (LIC 125) and discussed the purpose of today's visit. They guided analysts on a tour of the facility. This is a single-family home converted into a preschool center. The program operates Monday through Friday from 6am to 6pm. It offers care to children aged two to six years old. Present during today's inspection were licensee, two staff, and fifteen Children. Upon arrival, LPAs observed all required postings. All areas used by children were inspected for safety. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. The facility utilizes a wall heater for heating. LPAs advised licensee to barricade and/or use a heater guard to block access to the heater. In the living room, LPA observed operable water cooler in window. Per licensee, the windows in the home does not open. Lighting was observed to be in operable condition. LPAs observed age-appropriate toys. Children have their own cubby to store their belongings. Cots are available to children for napping. Per licensee, the isolation area is in the back room. Children who are sick utilize restroom inside of back room. LPAs observed an appropriate number of toilets and sinks available for children. At approximately, 9:31am LPAs observed water inside of a bucket underneath one of the sinks in the bathroom. The bucket was catching the water from a flexible stainless steel braided connector. Per licensee, the bucket is a temporary fix until the plumber comes to fix the sink. This is a potential health and safety risk Page 2 to persons in care. LPAs reminded licensee that water in a bucket should not be accessible to children. LPAs observed licensee remove the bucket. Per licensee, indoor and outdoor drinking water was available via kitchen sink and water fountain located in the hallway. LPAs verified that the lead testing was not completed and provided licensee with written directive outlined in Provider Information Notification (PIN) 21-21.1-CCP. This is a potential health and safety risk to persons in care. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Disinfectants and cleaning solutions were located on a shelf above toilet inside of bathroom in back room. LPAs observed medication in cabinet above countertop in the kitchen. Per licensee, no child currently enrolled require medication, but it is available if a child needs it. LPAs informed licensee of Incidental Medical Services (IMS) and advised them to refer to Best Practices Related to the Provision of IMS in Child Care Center\342\200\246 PIN 22-02-CCP. Poisons were not observed during visit. Per licensee, someone comes to fumigate every 4 or 5 months and they have one ant killer behind cleaning products in the bathroom. LPAs reminded licensee that poisons are to remain in an area that is locked and inaccessible to children. A carbon monoxide detector and smoke alarm were observed and are operable. LPAs did not observe a fire alarm pull stations. Disaster drills were logged and available. The last drill was conducted on September 13. LPAs reminded licensee that drills are to be performed and logged every six months. LPAs observed a menu posted on parent board, but it did not have date on it. LPAs reminded licensee that copies of the menus as served shall be dated and kept on file for at least 30 days. LPAs inspected the food preparation area. The kitchen was observed to be free of litter. Food supplies were observed. The facility provides snacks and lunch. The outdoor playground area was evaluated. LPAs observed all areas around and under high climbing equipment and similar equipment are cushioned with sand. Per licensee, the sand is cleaned every Sunday. LPAs reminded licensee that sand shall be evaluated daily. LPAs observed a tree that can be used for Page 3 shade. Per licensee, they transport children sippy cups outside to provide outdoor drinking water. Per licensee, there are bodies of water on the premises. Children\342\200\231s Records were reviewed for completeness. During record review, LPAs observed that two out of two children\342\200\231s files were complete. Children's roster was reviewed, but it was missing information. LPAs reminded licensee to ensure that the roster is complete. Parents sign children in and out via sign in sheet. Staff records were reviewed. LPAs observed three out of three staff records. During staff record review, LPAs observed that two out of three staff were missing a current mandated reporter certificate and three out of three are missing proof of immunization against measles and pertussis. This is a potential health and safety risk to persons in care. Licensee\342\200\231s mandated reporter certificate expired 06/07/2024, staff 1 mandated reporter certificate expired on 06/08/2024. Licensee has completed Pediatric First Aid/CPR AED on 06/01/2024. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated LPAs referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. Three Type B deficiencies were cited during today\342\200\231s visit. Appeal rights and a notice of site visit was given. The notice of site visit must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Lourdez Muniz.

During record review LPAs did not observe a valid certificate in licnesee and staff 1's file. Per licensee, they did not do the training. which poses a potential health, safety or personal rights risk to persons in care.
