What the inspector wrote

On 2/7/2025, at 10:30 am Licensing Program Analyst (LPA) Carolyn Tuba conducted a case management inspection regarding an incident reported on 1/13/2025. LPA met with Child Development Supervisor, Mercedes Gomez. LPA observed a census of 29 children with 15 staff. An incident report was called in on 1/13/2025 and an Unusual Incident Report (UIR) was submitted confirming 4 cases of conjunctives (pink eye) that began on 1/8/2025 and on 1/13/2025. The Department of Health was notified and came to the facility to conduct a visit on January 30, 2025. According to Child Development Director, Rosie Ducoing when LPA spoke to her on 1/30/205 there had been 7 more confirmed cases as of 1/24/2025 and all the cases occurred in room 27. The Health Department conducted a visit and the Director will be forwarding the report to the LPA. All surfaces, toys, furniture and walls in room 27 has been disinfected by using bleach and water. The children have been encouraged with more hand washing and a well check is conducted each morning to see if any symptoms are displayed. A letter was given to all parents notifying them of the conjunctives outbreak. As of today, 2/7/2025 there have not been any more confirmed cases. Program Nurse, Carmen Robles followed up by informing the families of the exposure to and provided material on steps to take in case they exhibit like symptoms. Program custodial staff were instructed to clean, disinfect and sanitize the classroom,materials and play equipment. The incident was reported to the Department within the required 24 hours of occurrence. No citations were given during today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the. Child Development Supervisor, Mercedes Gomez. page 1 of 1
