What the inspector wrote

On 6/11/2025, Licensing Program Analyst (LPA) Carolyn Tuba conducted a case management inspection due to the facility having reported and submitted an Unusual Incident Report (UIR) of a child\342\200\231s personal rights were allegedly violated. LPA took a census of 35 children with 8 staff members at Erwin. LPA then drove to Flanner location to speak with the Director, Rosie Ducoing as she works off-site. The facility had reported the incident via telephone and submitted an Unusual Incident Report (UIR) via email on June 3, 2025. The incident was reported to the Department within the required 24 hours of occurrence. The incident occurred on June 3, 2025, where a Parent #1 (P1) spoke to Site Supervisor, Mercedes Gomez and disclosed that she had witnessed Staff #1 (S1) allegedly grab a child by the shoulder in a rough way and yelled at the child during hand washing inside the classroom. Site Supervisor, Gladys Vargas followed up with P1. According to the Director and the UIR, P1 could not provide names of the children or staff however was able to point out the staff member P1 was accusing. S1 and P1 was not available at the time of the visit and contact information was provided. Children were not interviewed during the visit due to not having names of specific children. LPA was unable to interview additional children due to time constraints. Director was unable to provide any written staff statements, as certain staff have been off for the summer break, but an internal investigation has begun and will continue as staff return. LPA received a copy of the parent\342\200\231s statement. No citations are being issued during today\342\200\231s inspection visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Rosie Ducoing.
