What the inspector wrote

On 6/16/2025, Licensing Program Analyst (LPA) Carolyn Tuba conducted a case management incident inspection due to the facility having self reported and submitted an Unusual Incident Report (UIR) of a child\342\200\231s personal rights that were allegedly violated. LPA met with Child Development Supervisor, Mercedes Gomez and obtained a census of 35 children with 10 staff members. The facility had reported the incident via telephone and submitted an Unusual Incident Report (UIR) via email on June 3, 2025. The incident was reported to the Department within the required 24 hours of occurrence. This visit is to continue the visit conducted by the LPA on 6/11/2025 to interview staff and children, as well as Child Development Supervisor, Mercedes Gomez. The incident pertains to an allegation that was disclosed to the Child Development Supervisor by Parent #1 (P1) on 6/3/2025. During this visit LPA interviewed Child Development Supervisor, Child #1 (C1) and #2 (C2). LPA also obtained the contact information for a staff member who was on summer break and the Child Care Facility roster. Additional staff were also not available due to the summer break. No citations are being issued during today\342\200\231s inspection visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Child Development Supervisor, Mercedes Gomez.
