What the inspector wrote

On 05/21/2026 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection. Upon arrival LPA met with Child Development Supervisor Gladys Vargas to whom the reason for the inspection was explained. LPA was led on a tour of the facility. The purpose of this visit was to ensure the facility was in compliance with Title 22 regulations and to obtain additional information regarding an incident that occurred on 05/08/2026. There were no children present on this day they were released early due to on-site staff development training day. Per Gladys Vargas there are 79 children enrolled in the program. On 5/08/2026 the department received an incident report via email from this facility pertaining to a child sustaining an injury in the outdoor area from Room A. The incident was reported to the Department within the required 24 hours of occurrence. The incident occurred on 05/08/2026 in the outdoor play yard at approximately 10:10am. During the inspection LPA conducted interviews with the staff that was present during the incident, and Child Development Supervisor. LPA obtained documents in the form of Facility Roster, Ouch Report, Head Injury Report, and Internal District Injury Report. Based on interviews conducted child #1 collided with child #2 during outdoor play time. Child #1 sustained a bloody nose; staff applied first aid and contacted parent. Child #1 was taken to seek medical attention and returned to daycare the following school day with no medical restrictions. No injuries were reported for child #2. Per Child Development Supervisor there were 18 children present with 3 staff including a 1 on 1 staff. Based on the information that was verified during the inspection and the information on the incident report, the occurrence was an accident and Licensing staff did not observe any type of deficiencies that warranted a citation. No deficiencies are being cited during today’s inspection. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars. Appeal rights were given. Exit interview was conducted and report was reviewed with Child Development Supervisor Gladys Vargas .
