What the inspector wrote

Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 11/19/2025 and was reported to the Department on 11/21/2025. Director had informed LPA S Li during a visit on 11/20/25 regarding incident. A risk assessment was conducted. LPA met with Director Evette Estrada. Reason for the visit was explained. There were 5 children and 2 staff. On 11/21/2025, an unusual incident report was made to the Department regarding an incident where a rodent activity was observed in the kitchen. Melon that was stored on the kitchen counter had large rodent bite marks at the base of the melon. Kitchen was closed immediately. All surfaces were cleaned and sanitized thoroughly. All exposed foods were discarded and all other foods moved into sealed containers. Licensed pest control was contacted and scheduled emergency service. Full inspection conducted by pest control. During todays visit, LPA interviewed director regarding the incident, reviewed pest control service papers, and inspected kitchen, traps, and repairs made to walls, counter baseboards and screened outside crawl space under the kitchen area. LPA obtained screen shots of messages from pest control and plans of service and follow up. Per Director, there has been no signs of rodent activity since cleaning, moving the food and patching up the holes. Director placed cabinet locks on cabinets used to store food. All pet food that were previously stored in kitchen for outdoor cats have been moved to an outdoor storage shed. During today’s visit, LPA advised Shawn (board member) to replace the door sweep on the kitchen back gate. LPA observed the door sweep is worn down and there is a small gap at the bottom left of door where rodent could fit through. Kitchen door was open while chef was doing meal prep. Kitchen gate was closed. Per director, pest control is returning on Friday 12/5/25 to check traps and remove them. *continued on next page... Based on information obtained during this visit, follow up is necessary regarding the incident reported. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director Evette Estrada.
