What the inspector wrote

Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Annual/Random inspection at the above facility on 01/29/26 at 09:25am. LPA met with facility representative Lourdes Grana. According to the facility representative, they cannot guide LPA on a tour of the facility due to providing care and supervision; however, the Head Start office was contacted to notify of the annual inspection. At approx. 09:39 a.m., LPA conducted a self-tour of the facility. This is a Head Start preschool program which is operated by Montebello Unified School District and is located on the premises of Greenwood Elementary School. Entrance to the facility is through Jacmar Dr. The program consists of two classrooms (Room 01 and Room 02) and operation hours are Monday \342\200\223 Friday from 08:15 a.m.\342\200\223 02:15 p.m. Facility representative Jackie Ellingson later arrived and assisted LPA with the rest of the inspection. Per facility representative, there are 34 children enrolled; however, there were 07 children absent. All areas identified on the facility sketch were inspected. The following staff was present during this inspection: Room 01:12 children with 03 staff and Room 02: 15 children with 02 staff. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. LPA observed the facility to be safe and sanitary conditions. Indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Drinking water was readily available indoors and outdoors. LPA observed classrooms a water fountain. According to the facility representative, while outdoors room #2 will provide disposable cups so children can drink water from the fountain. Per facility representative room #1 will take a water jug with disposable cups while children are outdoors. Children can freely drink water from the water fountains located indoors and outdoors. Page 1 of 4 LPA observed cots are used as napping equipment. LPA also observed all children have their own cubby. At this time, the "isolation area" is used as an isolation area. A cot is available for an ill child to rest on. LPA inspected the children's restroom and observed that the sinks and toilets are working properly. LPA also observed that the children's restroom is safe and in sanitary conditions. The facility has a small kitchen with a refrigerator and microwave that were observed to be in good condition. The cabinet under the sink has a child proof lock. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were in accessible to children. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects, and rodents. LPA observed all required forms/publications to be posted in the entrance of both classrooms. Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Facility provides breakfast, lunch, and snacks and food is provided by the elementary cafeteria. All food is protected from contamination. Facility representative stated that there are no weapons, firearms, or bodies of water on the premises. First Aid supplies are available, and LPA observed a current emergency disaster drill. Last drill documented was on 01/16/26. At 09:58 a.m., LPA observed there are two functioning carbon monoxide detectors. At approx. 10:00 a.m., LPA took a tour of the outdoor play area and inspected the outdoor play equipment for any lose or pointed parts. Outdoor area around and/or under climbing equipment have cushioning material to absorb a fall. LPA advised the facility representative to check with the American Standard Testing Materials for Playground Guidelines and/or to use the manufactures recommendations for the playground equipment in efforts to be more in line with safety guidelines. Sign in and out sheets were reviewed on the child plus application to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPA observed all children present were signed in. Children\342\200\231s roster was reviewed and observed to be current. Staff and children\342\200\231s records were reviewed. LPA observed 1 out of 5 staff files had an expired Mandated Reporter Training AB 1207. According to the facility representative, a substitute will be sent to the facility and S5 will go to the Head Start office to complete the Mandated Reporter Training AB1207. LPA provided a technical violation. 9 out of the 9 children\342\200\231s files were observed to be complete. Page 2 of 4 AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018, any person who works in a child care facility shall complete the training and renew the training every 2 years. LPA reviewed child 1 (C1) storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. Commonly Asked Questions about Child Care Centers and the ADA are available at: https://www.ada.gov/resources/child-care-centers/ Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.) LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. Page 3 of 4 At this time, the facility is in compliance with California Title 22 Regulations. Therefore, there are no deficiencies being issued today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative Lourdes Grana.
