What the inspector wrote

An unannounced Required Annual site inspection was conducted by Licensing Program Analyst (LPA) Bardo Baluyot. Upon arrival, LPA conducted a COVID risk self-assessment and met with Teacher, Julia Woods. The facility which consists of two portables: State Preschool and Full Day. State Preschool consisted of 11 children with 2 staff. Per Staff, Full Day Program stopped operating in portable #2 this past September and relocated students to various sites in the area. State preschool program currently operates from 8 AM to 12 PM in portable #1. Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Storage for children's belongings and an isolation area with a sink, toilet were inspected. Isolation areas are in the staff offices adjacent to each portable. Age appropriate sinks and toilets were inspected for availability and good repair. General sanitation was observed. Availability of indoor drinking water was observed in form of water jug and disposable cups. The smoke/carbon monoxide detector was tested and was operational. Outdoor area and equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade and fencing were inspected. Play area was inspected for hazards and inaccessibility to bodies of water. Drinking water is made available for children in the form of a water jug and disposable cups. Snack/lunch menus were reviewed and observed posted in the facility. Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Food preparation areas were toured for safety, cleanliness. A review of cleaning and food supply storage areas was made. First Aid supplies was available and observed to be complete. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Teacher child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met. Sign in and out sheets were reviewed and found to be accurate. No deficiencies were observed during today's visit. No Children or staff files reviewed during today's visit. Files are stored at Options State Preschool's main office located at 18455 E. Railroad St. City of Industry, CA 91748. (626) 854-3449. Exit interview was conducted with Teacher, Julia Woods. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
