Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Franchesca White arrived at the above facility for the purpose of an unannounced Case Management - Incident Inspection. LPA White announced the purpose of the visit and was granted entry into the facility by Site Supervisor DeAndra Johnson. There are 45 children enrolled, and 25 present at the time of inspection. All 12 staff have current criminal background clearance. The purpose today's visit is to follow up on an Unusual Incident Report received to the department on 2/5/2026. It was reported that a day care child tripped and fell bumping her lip on the plant bed. There was no bleeding or bruising from the fall. However, LPA White inspected the area to ensure the safety of the play area, Interviewed site supervisor, and reviewed written statement by day care child's teacher. Based on observations, record review, interview, the facility is in compliance, and provided an incident report that captured the vantage point of the closest staff member to the child when the incident occurred. LPA White also observed the day care child present in class during the time of inspection. LPA White also conducted a phone interview with child's mother and determined that the day care child was physically fine, and that mother was comfortable with the child continuing in the classroom with respect to ensuring that supervision is always provided for the children at all times. LPA White reassured the parent that staff may not always be able to prevent a fall from happening when children are playing, but supervision includes being able to report how an incident may have occurred even if an incident can not be prevented. Parent understood, and agreed to participate in observation opportunities, and parent meetings to voice concerns as well as be a part of conversations that lead to solutions that are in the best interest of the children. Based on observations, interviews, and record review there will be no deficiencies cited in accordance with California Title 22 Regulations. (Report Continues 1 of 2 Pages ) Report was reviewed with Site Supervisor DeAndra Johnson. A Notice of Site Visit was given and must remain posted for 30 days.
