What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted an unannounced Case Management Incident visit at Hawaiian Head Start to follow up on a self-reported incident that occurred on 09/15/2025. The Monterey Park Southwest Office received the writing report on 09/15/2025. LPA met with teacher Veronica Martinez then toured the facility. LPA observed children napping. No concerns with care and supervision and ratio. LPA completed the child's file review. LPA obtained child's document. Interviews conducted with staff and others. According to interviews conducted. On the day of the incident, there were 9 children with three staff. During outdoor play, S1 (See LIC811) observed C1 slipped on the steps while going up the play structure. C1 sustained a cut on the chin. Staff immediately comforted C1 and provided first aid. Parents were contacted in a timely manner. Child was taken to Urgent care and received 3 stitches. C1 kept home the entire week. Child returned to school without any doctor\342\200\231s restrictions. Although the play structure is appropriate, the steps observed to be worn out and slippery. To take precautionary measures and prevent future incidents from reoccurring. Staff suggested securing the steps with non-slip textures. Based on the available information, it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and the report was reviewed with the facility representative, Veronica Martinez.
