On 12/16/2025 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection. Upon arrival LPA met with Child Development Program Supervisor, Gabriel Munoz to whom the reason for the inspection was explained. LPA was led on a tour of the facility. The purpose of this inspection was to ensure the facility was in compliance with Title 22 regulations . Census was taken. LPA observed 24 children with 6 staff. On 12/11/2025 the department received an incident report via email from this facility due to a parent alleging their child\342\200\231s personal rights were violated by a staff while in care. This incident was reported to the facility on 12/11/2025. The reporting party did not specify the date of the alleged occurrence. The incident was reported to the Department within the required 24 hours of occurrence. During the inspection LPA conducted interviews with staff regarding the allegation. LPA reviewed staff files and file for child #1, sign in and out sheet for the month of December 2025. LPA obtained documentation's in the form of facility roster, copy of child #1\342\200\231s observation log, and other documentation's pertaining to the incident. Based on the information that was verified during the inspection and the information on the incident report, a subsequent follow up visit will be conducted at a later date as further information is needed. No deficiencies are being cited during today\342\200\231s inspection. The notice of site inspection must remain posted for a period of 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars. Appeal rights were given. Exit interview was conducted and report was reviewed with Child Development Program Director Gabriel Munoz.
