What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted an unannounced visit at the above licensed facility to follow up on a Case Management Incident which occurred on 5/8/2026. The Monterey Park Southwest Office received the written report on 5/8/2026. Upon arrival, LPA met with Head Teacher, Araceli Garcia and toured the facility. LPA completed a review of children's files. LPA obtained children's records and personnel reports. Based on the available information through records reviewed and interviewed conducted, on 5/8/26 there were 14 children with 3 staff. While outdoors, C1 attempted to kick a ball then fell. C1 sustained a scrap on the right elbow and first aid provided. Staff informed the parents of the incident. Staff assessed C1 and there were no signs of pain or discomfort. On 5/11/26, parents reported to school that child had a fractured elbow and required to wear a cast. Per staff, care plan was in place upon child's return to school. No further action is required regarding this incident. No deficiency was found during today's inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the facility representative, Araceli Garcia.
