What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted an unannounced Case Management Incident visit at Creative Beginnings Preschool to follow up on a self-reported incident that occurred on 12/30/2025. The Monterey Park Southwest Office received the writing report on 12/30/2025. LPA met with Center Director, Liane D'Arezzo. LPA completed staff and child's file review. LPA obtained child's document and personnel report. LPA conducted interviews with staff and others. According to the record reviewed and interviews conducted. On the day of the incident, there were 15 children with 4 staff. Per staff, during outdoor play C1 was on the monkey bars then child let go of the bar then fell on the rubber mulch. C1 sustained a minor cut on the lower lip. Staff provided first aid and contacted the child's parents immediately. C1 was taken to family clinic and treated with skin adhesive. C1 returned to school without any medical restrictions. At this time, based on the available information, it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and the report was reviewed with the facility representative, Liane D'Arezzo
