What the inspector wrote

Licensing Program Analyst (LPA) T. Tran conducted an unannounced Case Management Incident visit at Creative Beginnings Infant Center to follow up on a self-reported incident that occurred on 07/18/2025. The Monterey Park Southwest Office received the writing report on 07/18/2025. LPA met with Center Director, Julia Flores and Program Director, Cristina Hinojosa then we toured the facility. LPA observed proper care and supervision. LPA completed child and staff\342\200\231s files review. LPA obtained child's documents, Children\342\200\231s attendance sheet, and personnel report. LPA conducted interviews with staff and others. According to the record reviewed and interviews conducted. On the day of the incident, there were 10 children with three staff. The incident occurred when staff opened the door to receive a child, C1 placed the fingers in the door space and staff closed the door. When the staff heard C1 was crying, she immediately stopped the door from closing. However, C1 sustained an open cut on the fingers of the left hand and medical attention required. Staff informed parent in a timely manner. Child was taken to the doctor. Per mom, the fingers were fine just need to keep the wounds clean. At this time, based on the available information, it does not appear this incident was the result of a Title 22 violation of lack of care and supervision. No further action is required for this incident. No deficiency was cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and the report was reviewed with the facility representative, Julia Flores.
