What does Type A vs Type B mean?
Type A. Serious citation. Imminent or substantial risk to children. The regulator requires corrective action immediately and may impose a civil penalty.
Type B. Lower-severity citation. Corrective action required, no imminent risk. The regulator monitors compliance on the next visit.

Health-Related ServicesIn the case of less serious injuries including, but not limited to, minor cuts, scratches and bites from other children requiring assessment and/or administration of first aid by staff, the licensee shall document the injury in the child's record and notify the child's authorized representative of the nature of the injury when the child is picked up from the center. This requirement was not met as evidenced by: Staff #4 admitted that the incident report dated 10/27/2021 was not given to the parent during pick up. Evidence has revealed that 2 out of 16 incident reports were not signed by parent/guardian, one was signed the following day. Three of the incidents required administration of first aid by staff. This poses a potential health and safety risk to the children in care.

To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement was not met as evidenced by: Evidenced have revealed that staff have told a child that the child will not get a cupcake if the child did not lay down and take a nap. This poses a potential health and safety risk to the children in care.
