What the inspector wrote

Licensing Program Analysts (LPAs) Jonnisha Culbert and Jeanette Estrada conducted an unannounced Case Management Incident Inspection. LPAs met with Director Mary Nichols and explained the purpose for the visit. The inspect ion was conducted to follow up on three individual incidents that occurred on 03/08/24, 06/05/24, 06/12/24 involving Child 1 (C1), Child 2 (C2), and Child 3 (C3). Director provided LPAs with a tour of the facility. During nap time, LPAs observed 71 children and 6 teachers. Director showed LPAs the location where each incident occurred. While walking in the classroom C1 tripped over their feet and fell into the play refrigerator. C1 hit ant their left ear on the refrigerator. Staff 1 witnessed incident and applied pressure and paper towels to cut. Facility followed procedures and notified parent who was located in the facility at the time incident occurred. Parent took C1 to Urgent Care where they received stitches to the left ear. C1 returned to school the next day. While walking towards a toy in the classroom, C2 tripped over shoes and fell and hit their right eye on middle board of book shelve. Staff 2 and Director cleaned the cut, added pressure, and an ice pack to stop bleeding. Facility followed procedures and notified parents who picked up C2 and took them to urgent Care. C2 received stitches and returned to school on 06/07/24 with no restrictions. During transition from outside play to indoors, C3 jumped on bench and onto table. Staff 3 witnessedd C3 and told them to slowly get down. Staff 3 walked towards C3 to assist them off the table. C3 jumpe down off table and his feet hit the leg of table, and he landed with head on ground. Staff 3 noticed that C3's right eye was bleeding and provided tissues and pressure to cut. Staff 4 helped Staff 3 with first Aid. Facility followed procedures to notify parents and C3 was picked up by parent and taken to Urgent Care. C3's cut was glued and they did not return to school until 06/14/24. Based on interviews and observations there were no violations of Title 22 requirements at this time. Notice of site visit was given and must remain posted for 30 days. Exit interview was conduct and report was provided to Director, Mary Nichols.
