What the inspector wrote

On 02/24/2026 at approximately 10:46am, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced case management inspection at the facility noted above. LPA met with licensee, Mary Nichols and stated the purpose of today's visit. The purpose of the visit is to speak with licensee regarding the outdoor play structure. During LPA's visit on 02/12/2026, LPA informed licensee that they will return at later date regarding the structure. Present during today's visit were licensee, 38 children, and nine staff. The play structure includes a slide, stair, and various activities components. One of the activities is an approximately three feet long squiggly bar that is attached to the play structure. It ranges from around one to two feet from the ground with movable shapes on it. The activity is intended for children to slide the shapes back and forth along the bar, however, children have been observed, by staff, using it improperly by straddling the bar. This poses a potential safety risk to children in care. During today's visit, LPA toured the facility, interviewed licensee, and observed the activity components. Per licensee, the bar is scheduled to be removed. No deficiencies were cited during today's visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee, Mary Nichols.
