What the inspector wrote

On 05/19/2026, at approximately 07:50am Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced case management inspection at the facility noted above. LPA met with facility representative, Mary Nichols and explained the purpose of today’s visit. The purpose of this visit is to cite the facility for an incident that occurred on 03/18/2026. There were 35 children and eleven staff when visit began. On 03/18/2026 child 1 (C1) injured their finger when it became caught in the door. LPA conducted interviews with witnesses and made observations. Per witness, “C1 likes to open and close the door". Other witnesses stated that,"[C1] always closes and opens that door," and "[they] know it was becoming an issue with that particular [child] opening the door". LPA observed photos and documents showing that the door can no longer be opened (sealed) after C1's injury. As a result of the incident, C1 fractured their finger when it became caught in the door. A physician's report confirms that C1 sustained a fractured finger. Although witness statements indicated that C1 had been opening and closing the door prior to the incident, the door was not secured until after C1 sustained a fractured finger. This is an immediate risk to the health, safety, and personal rights to persons in care. LPA Jonnisha Culbert informed facility representative, Mary Nichols that this report dated 05/19/2026 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jonnisha Culbert informed the facility representative, Mary Nichols to provide a copy of this licensing report dated 05/19/2026 that documents any Type A citation to parents/guardians of all children Page 2 currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt Licensing Report (LIC 9224), or other written statement, must be placed in the child’s file for verification. The Notice of Site Visit was given and must be posted for 30 days. Appeal rights were provided and exit interview conducted with facility representative, Mary Nichols.
