What the inspector wrote

On May 5, 2025, Licensing Program Analyst's (LPAs) Monique Ayala and Diana Ortiz conducted an unannounced Case Management -Incident inspection at the above facility. LPAs were greeted by Assistant Director Angel Haili; Director Melissa Garcia arrived shortly after. Director, guided LPAs on a tour of the facility. LPAs observed 34 children in care with 4 staff. The purpose of the inspection is to follow up on an incident that was reported to the department on 04/24/2025, the incident occurred on 04/23/2025. The incident was reported timely to the department. The incident is a possible personal rights violation. During the inspection, LPAs interviewed Staff #1 (S1) to Staff #4 (S4), LPAs interviewed Child #2 (C2) to Child #4 (C4) and obtained a current facility roster. Child #1 (C1) was not interviewed as C1 was absent from the facility at the time of inspection. At this time above incident report requires further investigation, no deficiencies are being cited at this time. An exit interview was conducted and a copy of this report was provided to director, Melissa Garcia along with Appeal Rights. A Notice of Site Visit was provided and must be posted for 30 days.
