THIS REPORT WILL BE AMENDED DUE TO TYPE OF VISIT Licensing Program Analyst (LPA) Ashley Caldern conducted an unannounced annual / random inspection visit at the above facility. LPA met with La Doris Green / Licensee who allowed LPA entrance into the facility. LPA disclosed the purpose of today's visit. LPA was guided on a tour of the facility along side L.Green. Per Licensee, currently no infants enrolled and facility hours of operation are 7am to 6pm, Monday to Friday. This is a one-story home which consists of 4 bedrooms, 1 bathroom, kitchen, living room, garage, front yard and backyard (fenced). The children use the bathroom in the hallway, living room, and backyard. Per licensee, areas off limits to children and parents include: all the bedrooms, kitchen, garage, and front yard. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection: LPA reviewed required posted documentation located at the entrance of the facility. Facility License and Publication (PUB) 394- Notification of Parent Rights posted. Licensing Form (LIC) 9148- Earthquake Preparedness form was provided to post. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 07/2025. Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption (if applicable). (Pg 1/ cont..) LPA observed Licensee\342\200\231s Assistant and 8 children present in the back yard. Per Licensee, was inside cleaning while the children are outside. Kitchen was observed, kitchen is an off limit area. LPA recommended baby gate in kitchen area. Knives are stored in high cabinet, inaccessible to children in care. Chemicals were observed in kitchen, side yard and in living room ;Licensee stored chemicals being used up high and inaccessible during LPAs visit. Chemicals / Cleaning components located from the kitchen side yard area is inaccessible with (2) doors observed locked. The licensee provides food for children in care. Raid Ant and Roach Spray (poison) was observed locked in high out of reach cabinet in hallway. Fire place is located in the living room, screened off. Living room and back yard was observed with storage boxes and bins stored on top of each other, there is a limit space children can use due to boxes/storage, area needs to be safe and clean for children in care, pictures taken, deficiency cited. Backyard was observed to have no bodies of water on premises. Off limit bedrooms were observed locked or observed with baby door knobs. LPA observed (4) play yards per licensee used for preschool children who still fit in them. LPA advised to obtain parent authorization for consent allowing preschoolers to use play yards for napping equipment due to play yards are typically used for infants. LPA advised when caring for infants to purchase firm mattress for play yards, Per Licensee, does not plan to enroll any infants. One Bassinet and One infant bouncer observed Per Licensee uses it for children to play with their dolls only. No infants observed in facility and enrolled per Licensee. Declaration form LIC855 was filled out stating Licensee understands not to use items above for infants/children in care only for play time. LPA tested the smoke and carbon monoxide detectors, found operable when tested. Fire extinguisher service tag is within 1 year dated July 1, 2024. The home maintains telephone service via cell phone. There are toys and other age appropriate material available for children. Per Licensee there are no firearms or weapons stored in the home. Per Licensee, currently no children on medication. Children and staff record was reviewed. Licensee and staff assistant have mandated reporter training that is valid and Pediatric first aid and cpr that is valid. Staff Assistant missing MMR vaccination, deficiency cited. License interview was conducted. Child #1 and Child #2 file was not observed with Parent Right form LIC995, deficiency cited. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on today\342\200\231s visit, Per California Code of Regulations Title 22, the facility was given (3) deficiency, Type B, provided on LIC 809-D. A notice of site visit must remain posted for 30 days provided via hand written form due to printer issues. LPA due to printer issue will send appeal rights and reports from todays visit.
