What the inspector wrote

Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection to follow up on an incident that was reported to the Department on 10/08/2025. Upon arrival at 1:50pm, LPA met with Lead Teacher Sylvia Weisse, to whom the purpose of the inspection was explained. A tour of the facility was provided, and census was taken. There were 11 children with 2 staff members. On 10/07/2025, a child in care was using the restroom when they ran and slipped on a wet floor, hitting the back of their head on the floor. The injury resulted in the parents of the child taking them to Urgent Care after pick up. No bump or broken skin was observed and no injury diagnosis was given. Facility currently uses the boys and girls restrooms of the elementary school grounds that it resides on, so the enrolled children at the facility are not the only children who use the restrooms and share it with the school and additional programs on the grounds. First aid and an ice pack was provided to the child immediately after the incident. Child returned to care the next day without any limitations and has been normal since their return. Per Staff #1, they have reminded children in care that there is no running in the restroom and that they should be following all safety rules. Facility also spoke with custodian and elementary school principal to request additional cleaning around 2pm after children have been released from school. Per Staff #2, the custodian has let the facility know to ask them at any time if they need it to be cleaned and provided facility their direct phone number. Based on the information above, there was no violation of Title 22 regulations. LPA provided an Advisory Note to the facility as advisement of continual checking of the restroom before children in care use it. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Lead Teacher Sylvia Weisse, at 3:15pm. Copy of Report provided. END OF REPORT
