What the inspector wrote

Licensing Program Analysts (LPAs) Shushanik Safaryan and Nolan Tcheng conducted an unannounced Case Management-Incident inspection for the purpose of following up on an incident reported to the Department on 03/01/2024. Upon arrival at 12:50 pm , LPAs met with Site Director Lindsey Kraut , to whom the purpose of the inspection was explained. Executive Director Alegra Inganni came in later .Tour was provided of the facility. There were 25 children present during the time of inspection. Census was taken. There were 25 children with 3 staff members. On 03/01/2024, an unusual incident report was made to the department regarding an incident that involved a child feeling uncomfortable due to the staff member comments. Per Facility Representatives internal interviews were conducted and staff members employment was terminated as of March 4 .The facility reported this incident to the Department within the required 24 hours. During todays inspection , interviews conducted with 4 staff members , 2 adults and 1 child , along with the documents obtained from the facility. At this time no citation were issued . An exit Interview was conducted, with the Site Director Lindsay Kraut and a copy of this report along with Notice of Site visit and Appeal Rights was explained and provided on 03/07/2024. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Page 1 of 1
