What the inspector wrote

On April, 8 2025, at 1:10 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted unannounced case management inspection for the above facility. LPA was greeted by Site Supervisor, Lindsey Acquazzino (Kraut), who guided LPA on a tour of the facility. The purpose of this inspection is to follow up on a self-reported incident that occurred at the facility on 03/25/2025 . This is a school aged program that is on the premise of La Canada Elementary School. The facility consists of 2 classroom bungalows, 35 and 36. These two bungalows are located on the playground of the school, next to the cafeteria. LPA observed 26 children in care with 7 staff members. Brief summary of incident: Child #1 (C1) was standing on top of the ladder section of the playground apparatus and jumped off landing on their feet. While C1 jumped, C1 extended their arms out and hit right forearm on metal side bar above the ladder. C1 was taken to the Emergency Room on 3/25/2025 by their parent and discovered C1 broke their right forearm. During this inspection, LPA interviewed Staff #1 (S1), Staff #2 (S2) and Child #1 (C1). Based on interviews and observations conducted there was no corroborating information to determine that there was a lack of supervision during this incident. LPA toured the outside area where the incident occurred and did not observe any hazards. All reporting requirements were met . There are no deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days during facility\342\200\231s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
