What the inspector wrote

On August 20, 2025 at 9:30 am Licensing Program Analyst (LPA) Priscilla Ochoa conducted an unannounced Case Management - Other inspection at the above facility. LPA met with Site Supervisor, Laura Navarette who guided LPA on a tour of the facility. LPA observed 24 children in care along with 3 staff members. The purpose of this inspection is to provide an amended report for an Case Management - Incident inspection dated 04/30/25. The amended report reflects the removal of Type A citation as evidence based on observations and interviews, the staff did not ensure that the child was following the play smart rules that are posted at the play structure to ensure proper use as instructed by the play structure manufacture. Type A citation has been downgraded to Type B. Facility has been issued an updated LIC 809D. The following deficiencies were cited in accordance with Title 22 of the California Code of Regulations and Health & Safety Codes. Please see 809D dated 04/30/25 for documentation of deficiencies. A notice of site visit was given and must remain posted for 30 days during facility\342\200\231s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
