Inspector’s narrative

What the inspector wrote

On 05/21/2025 at 02:30 pm, Licensing Program Analyst (LPA), Carolyn Tuba conducted an unannounced POC (plan of correction) inspection to ensure the deficiency cited on 04/22/2025 during an annual visit has been corrected. LPA met with staff member, Stephanie Martinez. LPA observed 15 children and 2 staff during this visit. LPA also met with Director, Geraldine Shaughnessy. LPA observed copies of the Mandated Reporter Training certificates for all staff members of files reviewed during the annual visit on 4/22/2025. The certificates expire ranging from 4/23/2027, 4/28/2027, 4/30/2027 and 5/5/2027. LPA reminded staff member and Director that the mandated reporter training must be completed every 2 years. During this visit LPA remeasured the classroom to confirm the capacity of the space before a fire clearance is requested, however based on the square footage per child the space remains the same and the maximum capacity is 20 children based on measurements. The current fire clearance indicates a maximum capacity of 20 children as well. The classroom currently has a morning Elementary school day program and in the afternoon the facility offers afterschool care for the children in the community. The LPA consulted with both the staff member, Stephanie Martinez and the Director and explained that when the afterschool children arrive which is approximately 2:30 pm, and 2 additional days where children have early release at approximately 1:30 pm. The Elementary school children if staying would then be enrolled in the school-age program. LPA also advised it is best practice to notify parents, so they are aware of the change. The staff member disclosed that the summer program will begin next week, and the children will be part of the school-age summer program. Those children would be required to have all licensing documentation in their files, as well as proof of immunizations since the children do not attend an elementary school outside of this day program. Children will need to be signed in and out by parents as required. The facility is required to page 1 of 2 adhere to all Title 22 regulations. LPA confirmed with the staff member that any staff working in the school-age program is required to meet all School-Age Child Care Center Teacher Qualifications and Duties, immunizations (MMR, Tdap, TB and influenza or waiver for the influenza), Mandated Reporter Training and the Pediatric CPR/1 st Aid EMSA approved is optional as long as they are not the opening or closing staff or left alone with the children in care. LPA cleared the deficiency on this date and provided a copy of the Licensing Report to staff member and issued a POC clearance letter during the visit. At this time, the facility is in compliance with California Code of Regulations Title 22. Therefore, no deficiencies are being cited. A notice of site visit was given must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Geraldine Shaughnessy. page 2 of 2
