What Happened
The complaint was substantiated that children were attending without properly maintained enrollment records. The roster and file information were inconsistent, and the investigation confirmed at least one child’s enrollment history was recorded incorrectly.
Observations
- One child did not have a record on file at first, and another packet had been returned to the parent because the signature was unclear.
- The roster listed children as enrolled in September 2023 even though one child had actually attended earlier and another had recently started.
