Inspector’s narrative

What the inspector wrote

Allegation regarding- Children attending the facility are not enrolled. According to the complainant, C1 and C2 do not have a file at the facility and feels that this could be a risk in case of an Emergency. RP also alleges that C1 and C2 are not on the children’s roster. Based on records review, C1 was enrolled July 2021; however, C2 did not have a record. Per S1, C2 does have a complete enrollment packet; however, it was returned to the parent because it was not signed clearly. Based on the roster provided, C1 and C2 were enrolled September 2023. Interviews conducted revealed that C1 did in fact attend in July and not in September as the roster stated and that C2 recently started attending. Based on record review and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, the following deficiencies are being cited (see attached 9099D).
