What the inspector wrote

4/30/26, Licensing Program Analyst (LPA) T. Tran conducted an unannounced visit at El Rancho Santa Gertrudes Center to conduct a Case Management Incident occurred on 4/14/2026. The Monterey Park Southwest Office received the writing report on 4/14/2026. Upon arrival, LPA met with teacher Maria Eva Villalobos-Mora and explained the purpose of today's visit. We toured the facility and observed proper care and supervision. LPA completed a review of children's and staff files. Documentation for the child and personnel reports were obtained. Interviews were conducted with staff and others. Based on the available information, there were 12 children with two staff members when the incident happened. During outdoor play, a child threw a plastic balancing block then hit C1 (see LIC811) on the left eyebrow. C1 sustained a minor cut and was treated with superficial glue. Child returned to school the next day without doctor's restrictions. No further action is required for this incident. No deficiency was found during today's inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Maria Eva Villalobos-Mora.
