What the inspector wrote

On January 29, 2026, at 1:45 pm, Licensing Program Analyst (LPA) Priscilla Ochoa conducted an unannounced Case Management inspection due to an incident that was reported to the Department on 12/10/2025. LPA met with Director, Danielle Svihovec-Mesquita who guided LPA on a tour of the facility. This is a preschool program which operates Monday through Friday from 7:30 am to 6:00 pm. LPA observed 31 preschool aged children and 5 staff members. On December 10, 2025, it was self-reported to the department that the facility had 4 children with cases of Hand Foot and Mouth Disease (HFMD), Child 1 (C1) - Child 4 (C4). All 4 children were in the Bears classroom. Per director, on 12/10/2025, the lead teacher in the Bears classroom, S1 observed spots on C1\342\200\231s knees and other parts of their body. S1 started to inspect all the children in the classroom. C2 and C3 we also observed to have noticeable spots on their body that aligned with HFMD. C1 \342\200\223 C3 were isolated in the director\342\200\231s office while parents were contacted to pick up their child because they potentially have HFMD. On 12/10/2025, the facility sent on exposure notice via Brightwheel to the parents of the children who are enrolled in the Bears classroom and provided them with resources on what to look for when inspecting their children for HFMD symptoms. The parent of C4 replied to the exposure notification to report that C4 had a confirmed case of HFMD. The director stated, they also Los Angeles County Public Health Department for guidance on HFMD cases. The facility\342\200\231s janitorial service conducted a deep cleaning of the Bears classroom on 12/10/2025 and conducted a deep clean of all classrooms on 12/13/2025. Staff continued to follow their sanitation procedures to ensure all areas were kept clean. Page 1 of 2 Per director, C4 has not attended the facility as of 12/02/2025. C4 returned to the facility on 12/15/2025, C1 returned to the facility on 12/22/2025, C2 and C3 returned to the facility on 01/05/2026 with no HFMD symptoms. Director confirmed that there were no additional confirmed cases. LPA provided and reviewed public health resources with director to ensure preparedness for any future communicable disease exposures. LPA confirmed that the facility is in compliance with title 22 regulations and health and safety code. No deficiencies being cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Danielle Svihovec-Mesquita. Page 2 of 2
