Correction check·June 3, 2026

On June 3, 2026 at 8:30 am Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced Case Management Inspection – Plan of Correction at the above facility. LPA met with Director, Danielle Svihovec-Mesquita who provided a tour of the facility. LPA observed 39 children in care along with 7 staff members. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 05/26/2026 were corrected. Licensing staff observed and reviewed the following: The facility has obtained the original box for the epinephrine injector with the label where it identifies that the medication has been prescribed to C1 by a physician Written instructions from the child’s authorized representative for the administration of the medication for C1 was obtained Letters of Deficiencies Citations Cleared were provided for deficiencies corrected. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director, Danielle Svihovec-Mesquita. Page 1 of 1
