On 10/22/2025 Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced case management -Lead Testing Exceedance inspection at the facility noted above. LPA met Facility Representative and disclosed the purpose of today\342\200\231s visit. The purpose of the visit is to address a water lead exceedance on the fountain in the classroom. At approximately 9:40am Facility Representative arrived at the facility. On 10/20/2025, facility representative contacted LPA J. Culbert to inform them of lead exceedance on one of the fountains at the facility. Today, facility representative provided LPA with the lead test results. LPA observed that there was a lead exceedance on drinking fountain C. The fountain was at the back of the classroom. It had a lead measurement value of 21 parts per billion (ppb). This was an immediate health and safety risk to persons in care. Per facility representative, on Friday (10/17/2025) the fountain was removed. LPA observed a faucet sink hole cover in the location where the fountain was previously installed. LPA provided facility representative with information on Provider Pin Notification (PIN) 21-21.1-CCP and informed them that, \342\200\234result[s] with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. LPA collected a copy of the water lead testing results but did not collect a copy of External Water Sampler Self Certification form (LIC 9275), Child Care Sampling Checklist form (Lic 9276), and facility sketch fully labeled with locations of all water outlets. Per facility representative, they will obtain all documents and email them to LPA by 11/07/2025. LPA reminded facility representative that, within two weeks after a completed sampling, the above documents must be submitted to The Department. LPA Jonnisha Culbert informed facility representative that this report dated 10/22/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Page 2 Also, LPA Jonnisha Culbert informed the facility representative to provide a copy of this licensing report dated 10/22/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt Licensing Report (LIC 9224), or other written statement, must be placed in the child\342\200\231s file for verification.
