What the inspector wrote

Licensing Program Analyst (LPA) Crystal Green conducted an announced case management licensee initiated inspection to inspect and measure facility for capacity increase determination. Licensee is requesting to add (3) additional classroom space to the existing preschool license. Due to COVID- 19 precautionary measures were taken, individuals present during inspection wore appropriate personal protective equipment. Licensing staff met with Director Suzie Johnson and Assistant Director, Annaliese Miller. LPA observed this facility to be located on the same premises as the La Canada Congregational Church. This facility has an approved COVID-19 Emergency Waiver allowing usage of the additional classroom space located on the second floor of the facility. At 10:35 AM, LPA was provided a tour of the facility\342\200\231s existing licensed areas and the proposed additional classroom space. The facility was observed to be operating and a total of 70 children with 12 staff members were observed. Per Director, facility is requesting to add (3) additional classrooms and an outdoor nature center. The (3) additional classroom were observed to be located on the second floor of the facility. Per Director, the new classrooms will be called the Discovery Room, Zebras, and the Grizzles. LPA observed cots and age appropriate furniture. Individual storage space is available for each child located in each classroom. Toys were observed to be clean and safe. LPA observed the facility to have a dirty and clean bucket available to sanitize toys and equipment daily. The children were also observed to each have their own sippy cups available. There are smoke detectors and fire extinguishers located throughout the facility. LPA was guided through the outdoor activity areas. Per Director, the facility has expanded their outdoor space to include a new Outdoor Nature Center play area. There is adequate shade available via trees and canopy in outdoor play area . Facility provides AM and PM snacks and lunch for the enrolled children. No transportation is being conducted by the facility. As a COVID- 19 precautionary measures, Director states that parents drop off and pick up will be conducted in outdoor courtyard area of the church. Prior to entry temperature check and a COVID-19 pre-screening questionnaire is conducted by staff for all visitors and children. LPA observed the facility to have installed a new keypad gated entry way accessible from Foothill Blvd. Per Director, Parents of the children enrolled in the Zebra classroom utilizes this entry way and the same precautionary measures are conducted prior to entry. This facility provides Incidental Medical Services- IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. Based on measurements obtained today from the both indoors and outdoors, the new capacity for the preschool program will be one hundred twenty (120) preschoolers. An approved Fire Clearance has been received by the department. At 2:08pm, an exit interview was conducted with the Assistant Director. A copy of this report along with appeal rights and the Notice of Site Visit were provided to the Director.
