What the inspector wrote

On 01/21/26 at 12:30pm Licensing Program Analyst (LPA) Diana Ortiz conducted an unannounced annual inspection to the above facility. LPA met with Licensee, Mary Henry, to whom the reason for the visit was explained. LPA observed eight (8) children, and one (1) assistant present at the time of inspection. Per Licensee, there are 12 children currently enrolled. Hours of operation are from 6am-6pm Monday through Friday. The licensee was observed to be operating within the license capacity limitations at the time of inspection. LIC 126 Entrance Checklist was provided to Licensee. Facility License, Emergency Disaster Plan, Parent\342\200\231s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the entry way of the home. A current children\342\200\231s roster was available for review. Licensee states there is one adult and no children living in the home. All adults living in the home have obtained a criminal record clearance. This is a single-story individual home and consists of 3 bedrooms, 2 bathrooms, kitchen, dining room, laundry room, living room, enclosed patio converted into daycare room, garage, dog run, a back yard and a front area. Areas accessible to children were inspected as follows: Far back bedroom (used as an infant sleeping area), enclosed patio converted into daycare room, bathroom and back yard. Page 1 of 4 Areas off limits include as follows: Kitchen, dining room, living room, Two bedrooms, bathroom inside master bedroom, laundry room, garage and front area. During this visit, all areas identified on the facility sketch that are accessible for children to use were toured and inspected. Licensee states that she is part of a food program called Angels, and provides the following meals: Breakfast, Lunch and AM/PM snack. Parents have a choice to either provide baby food or facility will also provide, and formula is provided the parent. LPA advised the Licensee any baby food, formula or breast milk that is provided by parents must be labeled with child\342\200\231s name with the date and stored appropriately. Per Licensee, there is central air and heating in the home. LPA observed a fireplace in the living room that is screened and has a safety latch making it inaccessible to children. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Poisons are locked in the off-limits area. LPA advised that any poisons should be locked under key or combination lock. Licensee does not have firearms in the home. The valve on the required 2A 10BC fire extinguisher indicates fully charged. Smoke and carbon monoxide detectors were tested and were operable on this day. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 10/15/25. The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children. Appropriate sleeping arrangements in form of playpens and mats were observed. Licensee states they are currently caring for three (3) infants. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and required infant\342\200\231s needs and service plan are on file. Inspection of required forms was made and documented on LIC 857. LPA observed 15-minute sleep logs to be consistent in checking every 15 minutes. LPA observed each child enrolled has an LIC 9227 on file. Page 2 of 4 Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys such as a riding and push cars, trikes, a playhouse, and other materials for children to play with. LPA observed that the Licensee does have proof of pertussis, measles and influenza immunizations, and the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. LPA issued an LIC 857, Children\342\200\231s Record Review, and LIC 859, Staff Record Review to the licensee which documents staff and children\342\200\231s files reviewed during this inspection. LPA observed one (1) dog (standard poodle) that is kept on the dog run during facility hours. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. LPA did not observe the following items during the inspection: Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family child care facility. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Mary Henry, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
