What the inspector wrote

Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management- incident inspection to the above facility. The purpose of this inspection was to follow-up on two incidents self reported to the department on 5/28/2025 and 6/11/2025. LPA met with Director Maricruz Flores- census was taken. On 5/28/2025 an incident was self reported to the department. Per Incident reported On 5/27/2025 child # 1 and # 2 were playing and pushing each other. Child # 2 pushed child # 1 causing child # 1 to bite their upper lip. On 6/11/2025 an incident was self reported to the department. Per Incident reported they had 3 confirmed cases of hand foot and mouth. LPA conducted interviews with staff Regarding incident # 1- incident was observed by staff- staff reminded children to use safe hands and not push each other. Child # 1 was taken to the office- area was cleaned and ice pack was applied. Child was not taken to the doctor. Regarding incident # 2 they had a total of 5 confirmed hand foot and mouth cases. All cases were reported to the Health Department. Facility has been cleaned and sanitized. All children have been cleared to come back. At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Maricruz Flores. ---------------------- pg. 1 of 1 -----------------------
