What the inspector wrote

On October 1st 2025, Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Case Management inspection due to an Unusual Incident Report (UIR) that was reported to the Department. The purpose of the inspection was to obtain additional information regarding the incident reported to the Department. LPA met with Director, Maricruz Flores who guided LPA on a tour of the facility. LPA observed 55 children and 8 staff present during inspection. On September 24 th , 2025 an incident was self-reported to the Department via phone call by the facility who reported that a child had swallowed a penny and as a result required medical attention. Per UIR the child’s parent was notified to pick up the child. The child was taken into urgent care and medical attention was provided. Upon further investigation it was concluded that per S1 and S2, C1 had swallowed a penny while under the supervision of S1 and S2. During interviews, S1 made LPA aware of cleaning protocols which consisted of mopping the classrooms with a cleaning solution containing chemicals while the children are present, mopping around them during nap time. Based on interviews conducted and LPA observations the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. Page 1 of 2 LPA Mariah Aguirre informed Director Maricruz Flores that this report dated 10/01/2025 documents 2 Type A citations which shall be posted for 30 consecutive days as there are immediate risks to the health, safety, and personal rights of children in care. Also, LPA Mariah Aguirre informed the Director Maricruz Flrores to provide a copy of this licensing report dated 10/01/2025 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days during facility’s hours of operation. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. An exit interview was conducted, and a copy of the report was provided to Director, Maricruz Flores.     No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidence by: C1 was unsupervised by staff members in the classroom, resulting in an immediate risk to the child's health and safety.
