What the inspector wrote

On December 8th, 2025, Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced case management incident inspection at the above facility. This visit was in response to a self-reported unusual incident submitted to the Department on December 4th 2025. Upon arrival, LPA Mariah Aguirre met with Director, Maricruz Flores. LPA explained the purpose of the visit, and a walk through of the facility was conducted. During the inspection, LPA MAguirre observed 42 children in care with 6 staff, and the facility was found to be operating within the licensed capacity and meeting Title 22 required staff-to-child ratios. On December 04, 2025, the Department received an Unusual Incident Report (LIC 624) via email regarding an incident that occurred at the facility on a date that was unknown. Director Maricruz Flores was made aware of the incident by child's authorized representative on 12/03/2025. The facility reported the incident to the Department on 12/04/2025, within the required time frame, per Title 22 regulations. The incident involved Child #1, stating to Parent #1 that Staff #1 wiped their bottom hard at potty time. Staff interviewed stated staff involved does not assist with wiping children during potty time, and only supervises. Per Staff Interviews if assistance is needed, other classroom assistance help the children during potty time. Children interviewed stated they go to the bathroom by themselves, and do not require assistance. During today’s inspection, LPA MAguirre conducted staff interviews, children interviews, observed the facility environment and children's bathrooms and reviewed incident documentation. LPA MAguirre provided consultation to Director Maricruz Flores regarding keeping a possible restroom log stating when staff assist children with potty. Based on the information gathered, at this time there is no evidence to substantiate a violation of Title 22 regulations. No deficiencies are being cited. A Notice of Site Visit was issued and must remain posted for 30 consecutive days. An exit interview was conducted with the Director Maricruz Flores. A copy of this report along with Appeal Rights were given to Director Maricruz Flores.
