What the inspector wrote

On 05/06/2026 Licensing Program Analyst (LPA) Mariah Aguirre conducted an unnanounced Case Management Deficiency Inspection to the above facility. Upon arrival LPA met with Director Maricruz Flores to whom the reason for the visit was explained and who guided LPA on a tour of the facility. During the visit, LPA observed 50 children with 5 staff members present. Based on information obtained during staff interviews, the facility had three separate incidents occur in April of 2026 that were not reported in the required time frame indicated in section 101212(d) which possesses a potential health, safety, and personal rights risk to children in care. Based on Information obtained, the following deficiency listed on the attached LIC809D (deficiency page) are being cited in accordance with California Code of Regulation Title 22. Deficiencies that are being cited meed to be cleared to protect the health and safety of children in care. The Notice of Site Visit must remain posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100.00.
