What the inspector wrote

On April 30,2025, at 2:30 pm Licensing Program Analysts (LPAs) Carolyn Tuba and Diana Otiz conducted an unannounced required inspection. LPAs met with Educational Coordinator, Seidy Ortiz and Director, Hytoshon Hodges-McDaniel who guided LPAs on a tour to gain a census of 37 children and 4 staff members in care. This is a preschool age program licensed for 84 children which operates Monday through Friday from 7:30 AM to 6:00 PM. Per Director there are 90 children enrolled who attend with various schedules. The following classrooms were inspected, Classroom 1 (2 to 2 1/2-year-olds), Classroom 2 (2 1/2 to 3-year-olds), Classroom 3 (3 1/2-4-year-olds), Classroom 4 (3-3 1/2-year-olds), Classroom 5 (4 - 41/2-year-olds), Classroom 6 (4 1/2-5-year-olds), Classroom 7 (Transitional Kindergarten (TK) (5 years old). Teacher-child ratios were observed to be in accordance with Title 22 Regulations. All children were observed to be under supervision, including visual. Licensed facility is within the conditions, limitations, and capacity specified on the license. Classroom 1 and 2 which are connected, share 1 toilet and 1 sink and LPAs reminded Director that those classrooms 1 & 2 can accommodate a total of 15 children combined. Classrooms 3-7 each have 1 toilet and 1 sink. LPAs observed inside of classroom 7 the sink according to Director, is ADA compliant for wheel chair accessibility. In classroom 3 LPAs observed a pet turtle that is kept inside a glass aquarium. During the visit LPAs observed a loft that was in classroom 6. According to the Director the classroom and the loft was included during a visit that occurred in 2004. The loft was part of the measurements and according to the staff member only 5 children are allowed with teacher supervision. LPAs took photos and stated that further review of the file need to be conducted. The LPAs observed that the staircase that lead to page 1 of 4 the loft has a gate at the bottom and another at the top that is secured. LPAs observed a spiral staircase. LPAs mentioned that a waiver may need to be requested but would have to review with their supervisor. The facility provides Lunch, AM & PM snacks and is provided by food service from the Foothill Country Day School which is an Elementary school (K- 8th) and share the same campus. Drinking water is brought from home and a filtered water station is available for refills as needed. LPAs observed menus posted in the classroom parent board. The kitchen was inspected and was found to be clean and in order. LPAs observed that there are smoke detectors that are connected to the elementary school and are tested throughout the year. LPAs observed and tested carbon monoxide detectors which were operable during the visit. LPAs observed fire extinguishers in each classroom that were last serviced on 11/26/2024. Emergency disaster drill was last conducted and documented April 2025. First aid kits are available in each classroom and in the sick room located in the lobby area where ill children will go until parents pick-up. Outdoor playgrounds are named the following: South, North, and East. The South Playground was inspected and LPAs observed artificial turf and included is a sand box that is kept covered when not in use. North playground has a play structure with wood chips to cushion the falls. East playground is used by the younger 2 year olds. The playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment have wood chips to cushion or prevent an injury to absorb a fall. There is adequate shade in the play yards with trees and shade cover. During the visit LPAs observed a swimming pool that is located next door to the preschool, however the pool belongs to the Foothill Country Day School. Per Director the preschool children do not use that area and LPAs observed that it is gated and locked. Director also stated that the pool area may be converted into a sitting/Pergola area for staff to take breaks. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. LPAs reminded Director that all parents are required to use full signature. Facility uses an app called ProCare and a code is required or QR code may be scanned for each parents/guardians dropping or picking up to sign-in/out their child. A report may be printed should LPAs request a copy. A current facility roster is available. page 2 of 4 Children\342\200\231s Records were reviewed to ensure that Identification and Emergency forms and a medical assessment are on file. Staff files were reviewed for Criminal Records Clearance for adults and verification of CPR/First Aid, Mandated Reporting Certificates were found to be complete. The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. The washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children appear to be toxic free. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. This facility provides Incidental Medical Services \342\200\223 IMS. LPAs reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP .
