What Happened
The preschool was generally in good condition, but the inspector noted an outdoor playhouse and picnic bench with cracked plastic edges and a shade issue that needed repair. Staff records were not available for review and were reported to be missing required immunization, mandated reporter, and CPR/First Aid documentation.
Observations
- The children's playhouse and picnic bench were cracked with sharp plastic edges.

Based on observation and record review, the licensee did not comply with the section cited above LPA observed the fire extinguisher needs to be serviced - expired 12/21/2022. LPA was unable to test the Carbon and Smoke Detector due to the service is linked together and needs to be serviced by a company which poses/posed a potential health, safety or personal rights risk to persons in care.
