What the inspector wrote

Licensing Program Analysts (LPAs) Alicia Mooberry and Tyler Reyes conducted a plan of correction inspection on this date. The purpose of this inspection was to follow up on cited deficiencies on 5/14/25 and ensure these deficiencies were corrected in a timely manner. Upon arrival LPAs observed 4 napping children and 1 infant. Also present was licensee's assistant Cheyanna Martinez. All adults present have been fingerprint cleared. During this visit LPA Mooberry observed 4 children napping on adequate individual cots in bedroom 1. LPA observed Licensee has removed the baby blanket and stated they understand safe sleep requirements. LPA observed uncleared Adult #1 was not present in the facility. LPA Reyes reviewed 4 children's files. LPA reminded licensee to ensure parents of children in care receive and sign LIC 9224 Acknowledgement form due to A deficiency cited on 5/14/25. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. LPA Mooberry found that the plan of corrections have been met for all cited deficiencies. Proof of correction letters were provided. No Deficiencies cited during this visit. Exit interview was conducted with Eva Hoffman, Licensee. Appeal Rights were discussed. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative . Failure to maintain posting as required will result in a civil penalty of $100.00.
