Routine inspection (3-year)·May 21, 2025

On 5/21/2025 at 8:50am, Licensing Program Analyst (LPA) Jonnisha Culbert attempted to conduct an unannounced Required 3-year inspection at the facility noted above. LPA met with Facility Representative discussed the purposes of the visit and provided them with an Entrance checklist for Child Care Centers (LIC 125). They guided LPA on a tour of the facility. This is a preschool center that utilizes four classrooms. The program operates Monday through Friday from 7am to 6pm. The program offers care to children two years old to six years old. Present during today's inspection were 33 children, and 8 staff. At approximately 9:30am, Licensee arrived at the facility. At approximately 11:14am, LPA observed a blotchy discoloration, ranging from gray to black, on the side of the lid of the outdoor drinking water container. Upon removing the lid, LPA observed that the discoloration extended across the entire interior surface. Per Licensee, the water is used to refill children’s water bottle when they are outside playing and container is cleaned every Friday. This is an immediate health and safety risk to persons in care. LPA advise Licensee to immediately remove container. During today’s inspection, LPA toured the facility and reviewed staff and children files. Due to time restraints, the Required 3-year visit will be completed on a future date. One type A deficiency was addressed during today’s visit. Other deficiencies observed will be addressed on a later date. LPA Jonnisha Culbert informed licensee, Grace Chin Lee Lin that this report dated 05/21/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Jonnisha Culbert informed the licensee, Grace Chin Lee Lin to provide a copy of this licensing Page 2 report dated 05/21/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt Licensing Report (LIC 9224), or other written statement, must be placed in the child’s file for verification. Appeal rights were given.
