What the inspector wrote

On 07/15/2025, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced Plan of Correction visit at the facility noted above. LPA met with facility representative and explained the purpose of today’s visit. The purpose of the visit is to follow up on a citation that was issued on 06/24/2025. Present during the visit were licensee, six staff and 36 children. On 05/21/2025, 06/16/2025, and 06/24/2025, LPA observed the bathroom floor in the diamond room was wet. Water was observed seeping from beneath the flooring, suggesting possible plumbing issue below the surface. Per facility representative, continuous flushing caused the water to come out in wet the floor. This was a potential health in safety risk to persons in care. Per facility representative, they will temporarily switch the children’s room and bathroom while they wait for the plumbing to be fixed in their current bathroom. LPA advised licensee that a plan of correction visit will be conducted to ensure that water no longer comes up from underneath flooring. Today LPA evaluated bathroom floor. LPA observed that previous flooring was replaced, and the new floor was dry. Per licensee, the flooring was replaced over holiday weekend and children returned to the classroom on 07/08/2025. LPA observed the flooring to be in adequate condition, with no visible signs of water pooling. During flushing of the toilet, no water emerged through the floorboards. No deficiencies were cited during today’s visit. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative, Grace Chin Lee Lin.
