What the inspector wrote

On 9/30/25 Licensing Program Analysts (LPAs) Portia Bowden and Javier Durnam conducted an unannounced Annual Inspection at the above facility. At 9:15AM LPAs met with Director Ali Sleiman, explained the reason for visit, provided entrance checklist (LIC 125) and were guided on a tour. Per Director the facility operating hours are Monday \342\200\223 Friday 730-230PM. LPAs observed all required documents on the facility licensing board (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A) LPA observed Emergency Drill log with a pre planned drill scheduled with for October 20 th , 2025. This is a preschool program consisting of 2 classrooms serving ages 3-4 and 1 enclosed outdoor play area. Classroom PK 1 cares for children aged 3, per Director there are 17 children enrolled. At 9:30AM LPAs observed 17 children in classroom PK 1 supervised by 1 Teacher Alma Farhat and 1 aide. Classroom PK2 cares for children aged 4, per director there are 17 children enrolled. At 9:35AM LPAs observed 14 children in care in classroom PK2 supervised by Teacher Soumaya Aldika and 1 aide. Facility also houses on the premises a K-12 Grade Private School. Capacity and limitations as specified on the license are being maintained. Per Director no children in care require Incidental Medical Services and facility does not provide IMS services. Per Director no children in care have food allergies. The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. At 11:00AM LPA observed a carbon monoxide detector on each classroom wall. LPAs observed cots for sleeping. Per Director cot linens for sleeping are sent home on Friday\342\200\231s to be washed by parents and then returned to the facility on Monday\342\200\231s. LPA observed 3 age appropriate toilets, 5 age appropriate sinks in good repair in the bathrooms located in the hallway. Per Director facility does not assist with potty training and diaper changing and requires all children to be fully potty trained upon enrollment. Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. LPAs did not observe Lead testing results. Per Director facility has not conducted required testing and was not aware of the requirement. A type B citation will be issued. Per Director facility uses filtered water bubblers inline with the buildings water line. Per Director children in care fill personal water bottles to take outdoors. LPAs observed Disinfectants and poisons stored in a locked janitor closet inaccessible to children in care. Per Director facility does not supply food or house a kitchen on the premises. Children in care are required to bring two snacks and a lunch. LPAs reminded Director to ensure all outside food brought in to the facility is labeled with child\342\200\231s name and dated. Outdoor play area was observed to be enclosed with playground equipment is in a safe condition, free of sharp, loose or pointed parts. At 10:00AM LPAs observed several age appropriate apparatuses with cushioned turf beneath. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPAs advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility. LPAs reviewed 5 staff files and observed 1 to be missing the required MMR immunization. Per Director staff does have the immunization and will print it and place in file. A Technical Violation will be issued. LPAs reviewed current Mandated Reporter certificates for all Staff LPA observed a Pediatric CPR/ First Aid for teacher Aldika Exp 10/25 and Teacher Alma Exp 9/27. LPA reviewed 4 Children\342\200\231s files and observed them to be complete. The name of the child care center director or fully-qualified teacher(s) designated to act in the director\342\200\231s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). CCC DID NOT COMPLETE TESTING PRIOR TO THEIR DEADLINE , LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. One Type B Deficiency and 1 Technical Violation were observed during today\342\200\231s inspection for lead testing and staff records. Exit interview conducted and report was reviewed with Director Ali Sleiman. A copy of this report, Appeal Rights, and A notice of site visit was given and must remain posted for 30 days
