What the inspector wrote

Case management inspection conducted by Licensing Program Analyst Jennifer Hua. LPA met with Site Supervisor, Christine Garcia. The purpose of the visit was announced. The purpose of the visit is to follow up on the incident that was reported on 12/7/2023. It was reported that on 12/6/2023, a child was left unattended on the playground during transition from outside to inside. During the visit, interviews conducted with site supervisor and staff. Staff stated that a child was left outside the classroom for about 1 minute, from the time the group went inside to the time staff found child sitting outside the classroom door. Based on information received, the following deficiency is cited on attached 809D. Upon receipt of this report documenting a substantiated complaint allegation and a Type A deficiency, the licensee shall do the following: 1. Post the Notice of Site visit and any licensing report documenting a Type \342\200\234A\342\200\235 deficiency. 2. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. 3. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). 4. The Acknowledgement form (LIC 9224) must be maintained in each child\342\200\231s file immediately upon receipt from parent. A copy of the parent Acknowledgement of Receipt of Licensing Reports Form was provided during this visit.     Responsibility For Providing Care and Supervision. The licensee shall provide care and supervision as necessary to meet the children's needs. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The requirement is not met as evidenced by: A child was left on the playground unattended during transition from outside to inside. This poses an immediate health and safety risk to children in care.
