What the inspector wrote

Licensing Program Analysts (LPAs) Susann Sanchez and Jeanette Estrada conducted an unannounced annual required inspection at the above facility at 11:40AM. LPA met with Monique Conner-Lewis, Licensee who guided analyst on a tour of the facility. There were 2 children and 1 infant present with the licensee, present when LPA arrived. Facility capacity is in compliance for a Large Family Child Care Home. Hours of operation are Mon-Fri 7:00 AM - 6:15PM. Food is provided by Licensee. Licensee was reminded if children bring food from home it must be labeled with the child\342\200\231s name and properly stored or refrigerated. Individuals residing in the home have been discussed and noted. This is a 2 story home which consists of 3 bedrooms, 2 1/2 bathroom, kitchen, dining room, living room, family room (fireplace locked), garage and backyard (fenced). The children have access to the bathroom near the living room, dining room, kitchen area and backyard as noted in the facility sketch. Per licensee, areas off limits to children and parents include: all upstairs rooms, upstairs bathrooms, and garage. The LPA toured all areas that would be used by children during the visit. There are weapons, firearms on the premises. LPA observed the firearms to be secured within Title 22 regulations. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection: LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 03/29/23. Smoke and carbon monoxide detectors were tested and are operable. The home maintains telephone service via landline and cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. LPA observed that detergents, cleaning compounds and medication are stored in the garage , inaccessible to children. Licensee states that there are no poisons stored in the home and understands that all poisons must be lock, not only inaccessible to children. Isolation area for sick children waiting to be picked up is in living room, away from the other children. Infant Care : Currently licensee has 1 infant enroll and 1 infant was present during inspection. Infant is almost 2 years old and currently sleeps on a cot. Napping equipment does not block entrances or exits. LPA observe licensee using the new Safe sleep regulations. LPA also reviewed the 15-minute sleep log. Children use the backyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA's did not observe any objects that could be hazardous to children in care. There are no pools or spas, or other bodies of water. Children\342\200\231s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunizations Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, LIC 9227 and the sleep log (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months). Staff records were reviewed including Pediatric First Aid and CPR certification, which expires on 01/25/2025. Other files reviewed were; LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement of Child Abuse. At 12:20pm during licensee file review, Licensee did not have Mandated Reporter training.Per Licensee was will complete of the training. Type B was cited. Licensee was reminded that all adults 18 and over, including employees and volunteers, expect as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be up assessed if this regulation is violated. During the exit interview Licensee M. Conner- Lewis, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. Based on the LPA\342\200\231s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22 Division 12, Chapter 1 and Section H & S Code. A notice of site visit was given and must remain posted for 30 days. Appeal Rights were given and explained. Exit interview conducted and report was reviewed with the Licensee, M. Conner-Lewis.
